Volunteer & Approved Drivers
Transportation to away games after school is done using volunteer parent drivers.
Parents are welcomed to drive their own child to games but there may be some parents that cannot drive their athlete to games.
Our program relies on volunteer drivers to transport athletes to the game and in some cases, back to Chaminade MS.
Parents that are interested in driving students other than their own child to games must submit a Volunteer Driver Form to Healthy Roster
Only "Approved Drivers" will be allowed to transport athletes to off-campus practices and games/matches.

Please click Volunteer Driver Form, print out and upload to Healthy Roster or email directly to Jo Bocanegra who handles all driver approvals.
Please understand that there is a 48-hour clearance process which includes a DMV check of the driver.

2023-2024 Healthy Roster Online Registration:
Chaminade will be using a new online program called Healthy Roster to register, fill out paperwork and pay sports fees.
After teams have been posted, parents will receive an invitation email from Healthy Roster to log in and create an account.
(Please check back by 3/4 to allow time for staff to update Healthy Roster if you don't have access)
After your account has been created, you will have access to upload the athletic forms required to participate in sports at Chaminade Middle School.

Healthy Roster is user-friendly and will remind you if you still have forms to complete.  With Healthy Roster, you will only need to submit some forms once while your athlete is at the middle school; others forms may need to be submitted annually.  After Middle School, your Healthy Roster account will move with your athlete right to our high school.  No new login or accounts required.

Families that participated in the Fall or Winter Sports Seasons already have an active Healthy Roster account.  All of the forms you submitted from the Fall and Winter seasons are good for the entire school year.  The only thing you will need to do is pay the Athletic Fees.
