I understand that all borrowed equipment is property of Chaminade College Preparatory. I agree to borrow the equipment described above for my personal use only and return them to the M.I.S. department in the same condition and at the agreed return date.

I understand that I am responsible for any damage or loss of any component of the equipment described above  according to the M.I.S. departmental policies. In case of damage or loss I will replace the equipment with equipment of equal value and functionality as approved by the Director of M.I.S.

I will not install any additional software or change in any way the configuration of the equipment.
